FEES & POLICIES

Fee Schedules: The Fee Schedule for all appointments is as follows:

Comprehensive Initial Visit: $385
Therapeutic Focused Consultation: $150
Follow-up Consultation: (based on $150 per hour (e.g. 1/2 hour = $75)
Acute Care Appointment: 15 minutes $40


General Policies Regarding Fees, Payments and Scheduling Payment:

The client is responsible for payment of all fees at the time of service. Payment may be made with cash, check, credit card or PayPal.

Insurance / FSA / HSA (Health Services Account):
If requested, we will provide account statements which clients may submit to their insurance companies or FSA / HSA managers for reimbursement. We do not file any forms for insurance or reimbursement. Clients are encouraged to investigate all options for insurance reimbursement or use of plans such as Flexible Spending Accounts and Health Savings Accounts which allow reimbursement of payments with pre-tax funds.


Cancellations:
If it is necessary to cancel or reschedule an appointment, notification of schedule change must be made at least 24 hours in advance. Missed appointments or late cancelled appointments represent time that was scheduled for the client and may be charged the full fee.
